Shockwave IVL: Cracking the complexities of below-the-knee disease
Below-the-knee (BTK) disease remains one of the most challenging areas in peripheral intervention, particularly in patients with chronic limb-threatening ischemia (CLTI), where heavy calcification and diffuse disease create significant unmet clinical needs. This CIRSE webinar, conducted in collaboration with Shockwave Medical, will review the pathology of BTK disease and current treatment limitations before introducing the role of Shockwave Intravascular Lithotripsy (IVL). Faculty will present the Shockwave E8 and JAVELIN platforms, supported by clinical evidence including BTK II data, and discuss how these technologies expand the BTK treatment armamentarium.
A focused deep dive into Shockwave E8 will highlight its technical features, its ability to address the vast majority of calcified BTK lesions and include two illustrative case experiences. The session will also define the role of JAVELIN for niche applications and conclude with a practical BTK treatment algorithm for daily practice, clarifying when and how E8 and JAVELIN should be used to optimize outcomes in complex BTK interventions.
Learning objectives
- Review the pathology of BTK disease in CLTI patients and identify key unmet clinical needs
- Present the role of Shockwave IVL in BTK interventions, including an overview of Shockwave E8 and JAVELIN supported by BTK II clinical data
- Explain the technical features and clinical application of Shockwave E8 in treating calcified BTK lesions, including case examples
- Define the role of JAVELIN in niche BTK applications
- Establish a practical BTK treatment algorithm outlining when to use E8 and JAVELIN in daily practice
