Abstract

Below the knee and pedal arterial disease are major causes of limb loss. Both endovascular and surgical revascularisation are efficient treatment options, therefore optimal therapy requires individualised decisions made by a multidisciplinary team. This course will provide an overview of infrapopliteal artery endovascular procedures, including patient selection, pre-procedural requirements, advanced tibial and pedal revascularisation techniques, post-procedural care and follow up. Below the knee and pedal arterial disease are major causes of limb loss. Both endovascular and surgical revascularisation are valid treatment options and individualised decisions for optimal therapy are required by a multidisciplinary team. This module gives all the necessary information on advanced infrapopliteal endovascular interventions, by providing an in-depth analysis of the pathophysiology, risk factor modification, treatment indications, endovascular recanalisation techniques, post-procedural patient management and follow up. Additionally, outcomes of below the knee and pedal angioplasty, including complications and their management, are discussed. After the completion of this module, the reader will be able to support the value and recognise the optimal setting required for advanced infrapopliteal endovascular treatment.

Learning objectives

  • List the risk factors associated with crural and pedal ischaemia.
  • Recognise the pathophysiology and anatomy of the infrapopliteal arterial system and its collateral pathways.
  • Describe the indications and contraindications for endovascular revascularisation and indicate the optimal treatment plan.
  • Arrange the set up necessary for infrapopliteal endovascular treatment.
  • Recognise the appropriate endovascular material in order to perform below the knee and pedal arch angioplasty. Demonstrate the appropriate technique for endovascular recanalisation.
  • Justify the performance of pedal arch angioplasty.
  • Arrange the necessary post-procedural care and follow up scheme.
  • Describe the outcomes and complications of BTK percutaneous revascularisation.
